Objective: Recruited virtually to enroll eligible participants in a blinded 2-arm, prospective randomized controlled study to understand the impact of Viora Health app-enabled, Educator supported program versus the status quo. Participants were enrolled after informed consent based on their risk scores Impact: The test group demonstrated statistically significantly greater interaction on the Viora app compared with the control group (76.1% vs 58.9%, p < 0.001), supporting the concept that the tech-enabled, human-supported Viora program can drive higher engagement than a primarily self-driven curriculum and RPM-based program. Recognition: The feasibility study and additional Phase 1 work on developing machine learning algorithms through this NIH grant resulted in a subsequent NIH Phase 2 grant for commercialize the
Objective: Enrolled eligible participants through community referrals in a Viora program for culturally competent care. Enrolled a mixed group of patients with an interest in general wellness and chronic disease management for a 10-month program to improve lifestyle metrics Impact: Patients reported improved sleep, stress, social support as well as clinical measures such as improved weight, blood glucose, and blood pressure levels. Additionally, they reported feeling supported through personalized recommendations on nutrition, exercise, and wellness plans and language proficiency to suit culturally familiar foods and practices. Recognition: Johnson & Johnson invited 6 of its alumni awardees to its headquarters in NJ to recognize them for their innovative and groundbreaking work in healthcare.
Objective: Recruited in Philadelphia to enroll eligible participants in a 6-month program to close social determinants of health gaps. Viora conducted social needs (SDOH) assessment to identify and design a personalized intervention plan (PIP ™) to resolve gaps in 4 key areas: food insecurity, health literacy, transportation, and social isolation. Enrolled patients reported having at least 1 of these 4 gaps. Impact: Patients were provided with appropriate connections within their own community (such as food banks to close food insecurity and senior centers to close gaps in social isolation) and connection loops were tracked and closed. 100% of social factors were reported as being resolved at the end of 6 months. Patients reported feeling more confident and supported by the program care team and peers without stigma or shame. Recognition: Submitted report to the American Heart Association for the Bernard J Tyson Social Impact Fund.
Objective: Received referrals from a community health center (CHC) to enroll eligible pre-diabetic participants in the National Diabetes Prevention Program (NDPP) based on their ADA risk score. Viora Program was composed of 26 virtual app-supported , educator-led group sessions over the course of one year. Impact: Patients attended on average 80% of all sessions and 75% of patients remained in the program at the end of one year. Patients lost weight, improved their activity minutes per week, and improved their blood pressure and A1C levels. They reported feeling more energized, and gaining the required structure and support to gain healthy habits and stay motivated over the year-long period. Participants formed a deep relationship-based community. Recognition: Submitted one year of weekly data to the CDC to gain CDC’s Full Recognition to Prevent and Delay Type 2 Diabetes credential.
